I reproduced it a couple of times, did also inventory reassignment (just as info). If two are visually stacked, rearranging the slots further leads to weird behavoir swapping other xenonauts not intendet. Looks like a an index missconfiguration in some for loop or something. Were you dragging the green icons around and then sometimes the icon underneath doesn't move, leaving two stacked on top of each other? I've tried that a few times and I can't seem to break it (although obviously your screenshot shows that it is possible to break it somehow).
