I have experienced this also in v12. I had an F-17 over top of the landing site, after clearing out the ground mission successfully it stated that the "ufo escaped to space" just like LeftyRighty mentioned.

Fired upon an alien to force suppression, and had a 0% chance to hit. Managed to hit the target despite the 0% which caused a crash to desktop. I have been able to reproduce this bug 3 times now.

Attempted to load a saved full game. Attempted 5 times with 2 different save files, resulting in a crash to desktop on every attempt.

Attempted firing a rocket from inside of a building through a window towards an alien outside of building. Rocket impacted with window as if hitting a solid wall. Immediately after this the game crashed to desktop. I'm not sure if it is because too many things were trying to happen at once, but that would be my guess. The game was processing 3 windows shattering, the wall cracking, the explosion happening, and the character that fired the rocket being killed all at once.
